You'll have to change it to Z visa by applying at Chinese embassy in your home country.

Only legal working visa in China is Z visa and all other Chinese entry visas makes you illegal when caught working.

If you find a job, the employer is now able to change the Q visa to a Z with work permit.  I just had this done.  Not sure if it is normally like this, but since travel is so difficult now and schools are desperate for teachers, rules have changed a bit.

I came in on a Q-2 visa.  I had already talked to a few different schools that wanted to hire me, but only if I was already in country.  I didn't want to come over on just a "maybe" job offer, but once I had serious interest from 3-4 schools, I got a Q-2 visa (wife is Chinese) and headed over.  I accepted a job in Beijing and the school did all the work on changing my Q-2 visa to Z and WP.  They even handled getting the necessary documents (background check, notarized docs).  I just gave them what they asked for and they took care of the rest.

Hotwater:

They changed your Q2 visa into a work based resident permit. The Z-visa is only a single entry visa the gets you into China. That’s then changed into a resident permit

You can't work with Q1 or Q2. Apply for Z visa.

A:Your potential employer is right! Purpose of the Medical exam before granting Z visa is to make sure, you won't be entering China in any abnormal and harmful to others condition.Few days after your arrival to China, you'll be required to complete another ME ... If ME in China will be OK, you'll get a Residence permit stamp into your book some 10-days afterwards. Imagine, you're an employer, who can sponsor my entry and work in China, but after my arrival to China would be found I carry some sickness, which would prevent Chinese .gov to issue a Residence permit. Rule is: Z visa should be converted into Residence permit in 30-days after entry to China.    So, my advice is:"Complete ME in your home country and forward medical results to your employer".Z visa grant will follow shortly afterwards. Employer is not wrong at their demand for the pre-arrival ME from your home country hospital.   -- icnif77
